Use SDC's secure upload service to submit supporting documentation for claim review. Up to eight (8) PDF, JPEG, GIF, PNG, MS Word or MS Excel files less than 20 MB may be uploaded in one submission. To send more files, please use a new secure upload form and include a note in the description field identifying the claim the files are associated with. If confirmation of receipt is required, please contact SDC’s Dentist and Member Services team at 937-438-0283 or 1-800-762-3159.
